What You'll Be Doing:

Providing On-Call Support: Perform initial nursing assessments for new admissions and provide essential care during on-call hours.
Creating Comprehensive Care Plans: Develop, coordinate, and revise total plans of care, collaborating with the care team.
Communicating Patient Progress: Regularly communicate patient progress to the clinical manager, physician, and care team.
Maintaining Accurate Records: Develop, prepare, and maintain individualized patient care progress records with accuracy and timeliness.
Managing Pain and Symptoms: Record pain/symptom management changes/outcomes as appropriate.
Educating Families: Educate families on patient conditions, needs, and care.
Making Referrals: Make referrals to other hospice care disciplines as needed, or document the rationale for not doing so.
Performing On-Call Responsibilities: Provide on-call services to patients and families as assigned.
What We're Looking For:

Current RN license specific to the state(s) you are assigned to work.
One+ year of nursing experience.
Current CPR certification.
Valid driver's license, reliable transportation, and liability insurance.

Work Schedule:

Monday through Thursday, 5:00 PM to 8:00 AM.
This is not a remote position.
