A community Primary Care Clinic near San Jose, CA is seeking a Licensed Social Worker to join their team. Details are listed below.

Pay Rate: $80,000 - $110,000 / Annually

Licensed Social Worker Opportunity:

Full-time, direct-hire position
Schedule: Monday – Friday, 8 AM – 5 PM
No on-call or after-hours work required
LCSW or LMFT will report to the Counseling Director

Licensed Social Worker Responsibilities:

The Licensed Social Worker will oversee case management by developing treatment plans and connecting clients with resources
The Licensed Social Worker works alongside psychiatrists, physicians, and other medical professionals to ensure clients receive the services they need
Help clients understand their own strengths and areas for growth
Assist clients in developing plans using their strengths to overcome obstacles

Licensed Social Worker Qualifications:

Master’s Degree in Social Work or related fields
LCSW or LMFT in CA Required
Bilingual in English and Spanish preferred
