Position Overview:
The Clinical Manager plays a key role in overseeing the day-to-day clinical operations of home care and hospice services. This position ensures that patients receive the highest quality care while collaborating closely with the leadership team to meet organizational and financial goals. You'll have the opportunity to lead a team, contribute to strategic initiatives, and make a real difference in patient care.

Key Responsibilities:

Conduct performance reviews and field supervision assessments for all clinical staff.
Coordinate staff vacation schedules and ensure appropriate coverage.
Oversee the orientation and onboarding of new clinical staff in collaboration with HR.
Collaborate with the leadership team to ensure home care and hospice programs meet or exceed financial goals.
Work with the intake and referral department to manage patient transitions from home care to hospice.
Provide oversight for patient care services, including coordination and patient assignments with scheduling teams.
Ensure patient care plans are updated, and that patient needs are consistently assessed.
Manage day-to-day coordination of field staff, ensuring efficient delivery of services.
Assist with quality management and continuous improvement of home care services.
Support the Quality Director in utilization reviews for home health patients.
Provide support and guidance to staff regarding patient care, including educating families on care plans.
Assist with skilled visits and participate in the on-call rotation as needed.
Lead interdisciplinary team meetings and contribute to staff training and development.
Expectations:

Full-time, exempt position requiring at least 80 hours per pay period, with additional hours as needed to meet patient and organizational needs.
Oversee clinical staff, social workers, and chaplains to ensure high-quality patient care.
Promote a positive and productive work environment that aligns with organizational values.

Direct supervision of clinical staff, social workers, and chaplains, ensuring they are supported in their roles and adhere to best practices.

Work Environment:

The position is primarily office-based but requires occasional field visits to patients. The role is eligible for remote work with manager approval.
Must be comfortable working in both clinical and administrative settings and proficient in using standard office equipment.
Physical Demands:

Primarily a sedentary role, but may involve occasional in-home visits that require light physical activity such as lifting files and standing.
Must have the ability to sit for extended periods while reviewing documents or entering data.
Travel Requirements:

Occasional travel within the service area is expected.
Qualifications:

Associate's degree in Nursing required; Bachelor's degree preferred.
Previous management experience is a plus.
Current RN license in good standing (NH).
Ability to work flexible hours, including weekends if needed.
Reliable transportation with valid driver's license and proof of car insurance.
Ability to meet lifting and physical requirements of the role.
Background checks will be required.
